You will typically need a complete ESA airline form to travel with your pet by air. An ESA airline form is free of charge for all Modern Animal members.

Please note that airlines establish their own policies for pet transport and adhere to the requirements of the federal, state, and local governments. Based on where you are heading, you may also need a domestic or international Health Certificate.  

Please note that Modern Animal is not able to provide any statement that your pet is a certified ESA. We can only verify your pet’s vaccines and overall health.
